G-639 for Deceased Italian grandfather, questions

G-639 for Deceased Italian grandfather, questions

Hi -

My grandfather was born in Sassano, Italy and came to the US when he was 2 years old.

According to my entire family, he never got US citizenship.

I'm trying to file a FOIA request, and I have a certified copy of his Death Certificate. On the G-639 PDF I checked off the "Deceased Subject"

But, what do I put in Boxes 3 and 7? Do I sign them myself or do I leave them blank?

Do I also need to include a copy of my fathers birth certificate to show that I am a descendant of the person's information I am requesting?
