right type of US birth certificate?

right type of US birth certificate?

I just received my birth certificate from my local health department. I requested a certified long form copy. I received a "certified transcript of birth" with raised seal including basic info such as time of birth, names of parents, registration number etc. This is not a certified photocopy of my birth certificate, which as more information on it.

When I used vital check to get my mother's BC, I noted the purpose for "dual citizenship" and they sent a photocopy of her BC with a raised seal.

my question is- do I need get get a certified photocopy of my BC rather than this extract?

It sounds as though VitalChek screwed up yet again and sent you a short form certificate. The consulate will not accept short form birth/marriage/death certificates.

Below are examples of a 1950's era New York long-form birth certificate and a recent short-form extract for the same person:

The extract you posted is what I received from my local health dept for my BC (he said the extract is the long form, which confused me). So this I need to change for the photocopy form, correct?

I don't think vital chek messed up, they sent what looks like what you posted as long form.

I often wonder if people get confused by the fact that (in New York anyway) the "long form" birth certificate is on a shorter piece of paper than the short form which is printed full size 8-12 x 11". :)

Of course, it is not about long form vs. short form, but about the amount of detailed information. The consulate especially wants to see the ages/birthdates and place of birth of the parents so that they can match these up to other documents. This information is not normally found on short form extracts/transcripts.

thanks carmine, i was wondering why there seemed to be so much confusion about it. your comment about NY seems to be correct.

HI Everyone,

Ok I too have a question now about the birth certificates. Everyone in my family was born in NY, my parents in the 1950's but they didn't give me a certificate like that. It does say the parents names, where they are from, and there jobs but it doesn't give information like the weight of the baby etc like the things I saw on this one. Please tell me these are the right certificates that I have!
