Detroit - green form?

Detroit - green form?

The Italian consulate in Boston tells me that because I was married in IN I need a "green" form from Detroit.
Anyone know what the "green" form is? The Boston Consulate has never heard of it.

There is no green form. Remember to send you application for marriage and your marriage license. Have both Apostilled.
