Name changes

I have mentioned this before, but need help in deciding how/what to change:

GGF: Filippo Listi
Birth Certificate shows Filippo Listi, but US documents show Philip Lester. My GF and F both were born "Lester",and my last name is Lester. I am hoping to meet with consulate late this year,and plan to show story as Filippo Listi, aka: Philip Lester.

I am having the death cerficate (shows Philip Lester) amended, and can only ADD information. My plan is to ask them to ADD aka: Filippo Listi, so it reads Philip Lester, aka: Filippo Listi.

My question: Is this okay? ...or would it be better to ADD Filippo Listi,aka:_________ (then reads: Filippo Listi, aka: Philip Lester), which may then cause them to ask me to amend my GF and F and MY documents that show Lester instead of Listi? Does it really even matter? Thanks for your help, again!
