Applying for a Codice Fiscale through NY Consulate

Does anyone know if, when filling out the form to get your tax code, you have to include your sample tax code produced from an online site or if you leave the form blank so the finance dept. can assign you one themselves?

Leave it blank; it will be properly calculated for you.
