My grandfather departed Naples in 1903 for Ellis Island. Can anyone please tell me what documents he would have needed to leave Italy? Passport, birth certificate, baptismal certificate, etc.?

Hello Leigh Ann :-) Welcome to the forum...

Emigrants from Italy would need a passport...
It is my understanding that they could not purchase a ticket without one...

After the unification of Italy (around 1870), Italian citizens were required by their govt to have passports. It was one of the ways the Italian govt had of controlling its citizens and insuring that any young males who left the country did not do so to avoid the draft there. In 1903, however, the U.S. did not require that an immigrant have a passport in order to enter this country.
