I am trying to find the birthplace of a relative Sisto Zaccari who was born in 1895. He came to America in May of 1920. The town he was living in at the time was Forentino. He came to America and married Anastasia Gorski. They had James, Jerome, John, Anthony. They resided in Berwyn Illinois.
Sisto Zaccari dies in 1962. Just before he died he went back to Italy to visit relatives. I seem to remember a town mentioned PALERMO but I don't know if that is where he was born. I also think he was born somewhere in Sicily but I'm not sure.. Sisto's mother's name was Chiara.

If his wife was Clementa and his Brother Giuseppe and Mariano going to Pittsfield, Mass, USA. There cousins were Calicchia's. He was born 1892, Ferentino, Frosinone, Lazio, Italy.

I don't see his death info in the online death index. His place of birth may have been on that. I don't know if it would be on his marriage record, or there would be any other info on that record you might need.

I agree but have been told that the application will often say simply "Italy". I have no direct knowledge, as I have never had the need to order an SS-5.
Assuming that the 1920 manifest is the correct one, it seems quite clear that place of birth (line 12, last column, page 2) was Comune di Ferentino, Provincia di Frosinone, Regione di Lazio, Central Italy

I have quite a few social security applications for ancestors, direct line and siblings (got them when it only cost $7.00 years ago). They all have town names on them. Not the same case though with WWI draft registration cards. I have found that the ones from the various boroughs of NYC only say Italy, but those from outside of NYC, such as towns in Westchester County, NY or upstate NY, give the name of the Italian birth town.
