Help reading ship manifest

I am hoping someone can take a look at the Ellis Island ship manifest, line 6 - for Pietro Bartolomeo. I can't figure out what it say's above and to the right of Dr. Certificate.

Also - Do you think entry of Dr. Certificate was for Bartolomeo or Spano on line 7?

It says Likely Public Charge. He is going to see his son Gaetano. I had a look and found that he was detained at Ellis for LPC.

You can keep going forward to the end of the roll to view the list.
This is how it was indexed on ancestry:

Name: Pietro Bartolomeo
Arrival Date: 28 Jan 1905
Birth Year: abt 1868
Age: 37
Gender: Male

On the top left corner of the manifest image you post above
is a note regarding Pietro Bartolomeo - After the notation "At ship's risk(crossed out) it then indicates "(Hernia)" - This is also the word written to the right of the notation "Dr. Certificate" at the end of line 6...
