Meta di Sorrento- merchant marine-trapani

I have a merchant marine certificate of my great grandfather, Salvatore Trapani who was born about Feb 25, 1853 in Meta di Sorrento and came to NYC approximately 1900 (have never been able to find his immigration record). I was wondering if anyone knows the significance of this merchant marine certificate (I have attached a copy). I understand part of it lists his parents- Giuseppe Trapani and Ratchel Cacase (I have the information from his death certificate), but was wondering what other information can be obtained.

Also- how do I go about obtaining his birth certificate from Italy?
