Translation help please - PASSAPORTO

Can someone please translate the following document for me? Note that there are four pages - two inside and two outside, whatever you have time to translate, I would appreciate... I'm also curious to see if Raffaela's maiden name is on the passport.

She is listed as "single" [nubile] - her maiden name is the one on her passport.
Even if she were married... she STILL would have traveled using her maiden name as Italian women did.

That is a beautiful photo on the inside.
Below it is a physical description of Raffaela
Height - average; eyes - brown/chestnut; nose - greek; mouth - correct; hair - black; color - rosy; build - correct;
