Death of Paolo Polizzi, 1634

I have 2 questions about this death record

1. Although I can tell that this is from 1634 AD, I can't decipher the actual date. I would really appreciate it if one of you could decipher it for me
2. Does anyone know why he would be referred to as "Mro Paolo di Polizzi" instead of just "Paolo Polizzi"?

August 31 1634
Mro = Mastro/Maestro was an honorary title given to people who "excelled" in some way.
Often the surname of a person indicated his origin, so it is very likely that Paolo's roots were in Polizzi, so his family was known as "di Polizzi", the surname eventually became just "Polizzi"

The one thing that makes the "di Polizzi" surname odd is that neither his father nor his grandfather was ever referred to as "di Polizzi", only as "Polizzi"

In that time frame surnames were very variable...

That makes sense, I have seen a lot of variant spellings (Ciolino/Ciaulino) for example
