Does anyone know exactly what this is?

I get the home page. Is that what you are referring to? The one that says Atto di Nascita and has a registration form? Since I have never been to their site before, I'm not sure what it should look like...

The Page heading:

DICHIARAZIONE SOSTITUTIVA DELLA CERTIFICAZIONE DI NASCITA

translates (roughly)... (from Altavista Babel Fish) to:

SUBSTITUTIVE DECLARATION OF THE CERTIFICATION OF BIRTH

It almost seems like maybe a way to register your birth or the birth of your child. I am probably wrong, but very intrigued.

No, is just a way to make a certification makink a declaration.
It came by the law n. 20 of 4 january 1968. The pourpouse is that, when you have to bring certification to some statal office, instead to go to angrafe and make the certificate you can do a substitutive declaration and after will be the office which will made controls if you tell the truth.
An italian way to try to eliminate burocracy, failed....
