Birth Act letter request

Hello,
I last posted about two years ago regarding my grandfather's birth certificate. At the time I was trying to get the film through the history centers but was unable to find his certificate through them. Since then I have tried other sources but with no luck. I would like to write to his commune and make the request from them. I have the address of the Servizi Demografici in Fagnano Castello(CS) but have no idea what is required in the body of the letter. Is there a template letter that I could use and would someone be kind enough to supply it to me?

The specifics are:
name: Emidio Sereno
birth: December 29,1888
commune: Fagnano Castello

Any help would be appreciated.

You can go to this website. They are form letters. Just fill in the blanks. It will translate to Italian. Copy the letter and paste into your word processing program and print. You can then save it as your record.

The form letters are great but you need to remeber to ask for the birth act, not the birth certificate.

The birth act contains more info..like the names of the parents and if you're really lucky, you may get the names of the grandparents (Usually the grandfather), the names of the witnesses and even where they lived.

The letter that I have composed using the form letter as a guide is asking for l'estratto dell'atto di nascita. Is the extract the full birth act document or should I just ask for the atto di nascita?

You need to ask for the
Estratto integrale dell'atto di nascita con annotazioni.
