As a surname, the name derives from the Italian word for pilgrim. As a first name, I really don't know what the English equivalent would be, but I do have someone in my family tree database whose Italian first name was Pellegrino and went under the name Paul. But that may have just been what family members decided to call him and not the actual English equivalent of the Italian first name. Paul in Italian is normally Paolo.
