Translation Request Vincenzo Montemurro- Marsicovetere

I'm requesting translation assistance for a Nascita(birth) record for Vincenzo Montemurro born in Marsicovetere 1858.
The image of the document has been posted on imageshack.

My questions are as follows:
1. Line 10. What is the Mother's name? Maria Rosa Surname?
2. Line 11-12 Father's name: Giovanni Montemurro
3. Line 15: what is the house description?
4. Line 17: Child's name Vincenzo Montemurro ________ what is the next name? Looks like Vincenzo again.
5. Line 14: nel giorno: what is the date of birth? 17 July 1858
6. Lines 19 & 21: what are the names of the witnesses?
7. Lines 4 & 5: who presented the child?
Any assistance would be greatly appreciated!

The record is dated on the July 19th and I believe the birth took place on the 18th.

Mariarosa's maiden name is Romagnano

The first witness was Domenicantonio Bove, the son of deceased Giuseppe, he was 37 and a peasant/farmer living in the town on Strada Maggiore (sp?), in a house without a number.

The second witness was a Clemente Cortese of deceased Bernardo (?), age 33 (?), strada Maggiore (?) also a house without a number. He was a shopowner.

yes, for some reason the name Vincenzo appears both before and after the infant's name in the record. I have no idea why.

The child was presented by a midwife. Her first name could be Teresa last name was Caso. She was the daughter of deceased Francesco and was age 47.

Baptism info is in the right column and it took place on the 19th.

the mother was age 26 and the father was age 30. I see the birth took place in their house which was located on Via ? I can't read the street name though.
