Need Translation Help with Italian Record Please

I need some help with translation of this birth record. I'm a little better at it when it's not all freehand. Here's what I can tell.

Can anyone please tell me the day/date of birth?
What is everyone's age here if it's listed?
Is there an address?

Any other relevant information out there?

You might have to put your images on imageshack.com because not everyone has ancestry subscription

As Marty mentioned, not everyone has Ancestry...

Even for those of us who do...your link only brings us to the Ancestry Home page...not to the record.
