I can't read the handwriting, but it seems like she had to pay taxes, so what is now called "Agenzia delle Entrate", let's say the Central Agency for Taxes (I couldn't say how it is called in the USA) wrote to the Commune of Napoli asking if she owned anything, and the Mayor answered she didn't.

It is a "Certificato di povertà" = poverty certificate. It was issued by the major of the town where the person was resident to allow him/her to not pay for some taxes. (I found many of these certificates in the passport requests to allow the petitioner of the passport to not pay for stamps or taxes)
