Death index column heading meanings

What do the three column headings mean?

The first column heading is
Cognomi e Nomi dei Genitori (last name and first name of the parents)

Next column is Giorno della morte (day of the death) but actually what you have is the day followed by the month followed by the year

I don't know what that last column says

I think the last column states "Osservazioni", that should include any particular note about the event.
