Please help with name!

I can't read the names on the envelope. I know it is from Forino, Avellino.

From: Nicola? (can't read)

fu or fa? Ca?

Nicola's surname starts with a capital I as in Italy. The surname is Iacuzio. It says fu (meaning deceased) Camillo, so Nicola Iacuzio was the son of a deceased Camillo Iacuzio.
