Dated June 29, 1892 at 9:30 a.m. in the town hall.

Appearing was Filomena Santonastaso, the daughter of Andrea (Santonastaso), age 30, a weaver living in Maddaloni. Also appearing was Marianna Tenneriello, daughter of deceased Alesandro (Tenneriello), age 40, peasant/farmer, living in the stated town. Both reported that at 8 a.m. yesterday, in the house on via Feudo (no number on the house), Raffaele Merola, 34 days old, died. He had been a resident in Maddaloni and had been born there. He was the son of Michele, a shoemaker, and Rosa Desiato, a housewife. Witnesses were Alesandro d'Aiello, son of deceased Domenico, age 74, and Giuseppe Pellegrino, son of deceased Michele, age 38. Both were residents in the town.

The document was read aloud to all who intervened. The witnesses and town official signed the document. The informants were illiterate and did not.
