Cipriano, Mea Ceriaco Birth

Numero 126 for a translation please. (not sure I have the correct spelling)

the word between Cipriano and Ciriaco is crossed out, the name is Ciriaco.
On Dec 20 1995 in front of the official appeared Nicola Maria Cipriano, aged 40, peasant farmer, who declared that, at 9:25 AM of the day 19, in Contrada Massariola #9, a baby male was born to his wife Grazia Stanco, the baby was named Ciriaco.
