I need help translating a few things from this document. Thank you so much!

Died at Via Piazza # ?
age 90, resident of Carlantino, born in ?
Father is deceased Nicola Perillo, in life lived in (same town as above ?)
Mother is deceased Rachela Rosa ? , in life lived in Carlantino
Widow of Giulio Pinto

age 90, resident of Carlantino, born in Calenza Valfortore (also a comune in Foggia)
Father is deceased Nicola Perillo, in life lived in Calenza Valfortore
Mother is deceased Rachele Tosa ? , in life lived in Carlantino
Widow of Giulio Pinto

I believe the mother is Rachele Tosa.

Thank you so much Angela! I was hoping that Tosa was her last name. It gives me a lead for her death certificate. I was hoping it did not say Rachela Rosa ( a middle name only)
