Help with a Short Letter to a Consulate in Italian

Ciao tutti,

My Italian, especially written, is only "cosí-cosí". Would you mind looking over this brief note and making sure I didn't make a fool of myself? Thanks in advance!! :D

Io e la mia famiglia abbiamo recentemente traslocato in la vostra giurisdizione. Siamo ammesso la cittadinanza italiana (jure sanguinis) in 2012 per il consolato in Filadelfia. So che ho bisogno di riempire una forma e presentarla al questo consolato. Voglio accertare che ho trovato la forma corretta nel suo website. Questo é la forma corretta?

(As my Italian might not be entirely correct, please allow me to offer this message also in English):

My family and I recently moved into your jurisdiction. We were granted Italian Citizenship (jure sanguinis) in 2012 through the Philadelphia consulate. I know I will need to fill out a form and submit it to your consulate and I want to make sure I found the correct form on your website. Is this the right form?

Io e la mia famiglia ci siamo recentemente trasferiti nella vostra giurisdizione. Abbiamo avuto la cittadinanza italiana (jure sanguinis) nel 2012 attraverso il Consolato di Filadelfia. So che devo presentare un modulo al vostro consolato e vorrei sapere se il modulo che ho trovato sul vostro sito è quello giusto.

ForzaItaliaPgh, I was recognized by the Philadelphia consulate and moved to Italy shortly thereafter. Nine years later, I returned to USA and moved into the San Francisco consulate's jurisdiction. I found the AIRE form on their website, filled it out and snail mailed it, afterwhich I received a telephone call at home from the San Francisco consulate to confirm that I was indeed at the address in their jurisdiction which I had specified on the form. So don't be surprised if you receive a similar call.
