Birth Record -can't read some words. Help!

Birth Record -can't read some words. Help!

Hi there-
This is my first foray into this forum for help. Any guidance or expertise is greatly appreciated!
If anyone is really fantastic and has lots of time on their hands a full translation would be SO incredible.

but if not... here are my really troublesome parts (the blanks are the words I can't decipher):

The first line: 'addi, ___________ di Aprile' what number is that? I understand Aprile of course.
Second line: 'a ore ______ meridiane' what word/number is there?

Lines 9/10 this whole sentence boggles me: 'Via S. Vincenzo al numero ___,da _?__?__?__?__?___'

Final line on first page and into the second page.

I guess that is most of the document actually! Ha! :oops:
