Help with letter to write to Postiglione,Salerno

I would like to write and or fax a letter asking for a birth act for Maria Giuseppina/Josephine) Peligrino. Could someone help me write that and let me know where to send it to and fax it? It would be much appreciated. Just trying to get more info. on her on where she might be born.
