Help writing a birth act request letter

Postby legnoduro » 05 May 2008, 01:42

Hello,
I last posted about two years ago regarding my grandfather's birth certificate. At the time I was trying to get the film through the history centers but was unable to find his certificate through them. Since then I have tried other sources but with no luck. I would like to write to his commune and make the request from them. I have the address of the Servizi Demografici in Fagnano Castello(CS) but have no idea what is required in the body of the letter. Is there a template letter that I could use and would someone be kind enough to supply it to me?

The specifics are:
name: Emidio Sereno
birth: December 29,1888
commune: Fagnano Castello

Any help would be appreciated.

Thank you.

(I originally posted this in the wrong forum. I tried to delete it and post it here but I was unable to do so. I hope I am not offending anyone by posting in this forum without deleting from the other forum.)
