Correct address for Casacanditella?

Correct address for Casacanditella?

Hello everyone :)
I am going to send for my grandmother's birth certificate. From everything I can gather, she was born in Casacanditella, province of Chieti. Is this the correct address that I would send my query:

Comune di Casacanditella
Via Marrucina 43
66010 Casacanditella CH

If this is correct, would I address the envelope as above?
Do I send money?
Do I include a self addressed envelope?
Do I ask for it by a specific name? I want whatever will have the most information.

I think they also married in Casacanditella. Could I request the marriage certificate at the same time or would it be better to send my request separately?

in Europe correct you write in this form :
I - 66010 Casacanditella
I in front of the postalcode and town shows the state/country (Italy).
for Germany would be D - , for Austria A - . for Switzerland CH - for Netherlands NL - and so on.....
Italy is english and not the correct form in Europe - if you will write it you better take ITALIA (italian name for Italy). always better to take the correct name the country has itself. not everywhere english can be understand. specially italians often are not fluent in english, in fact never learned.

when i write from Europe to USA i would never write in my german language "Vereinigte Staaten von Amerika". nobody in the states would understand. i would write USA like it´s usual there.
