Assuming you mean Termini Imerese, in Sicily, then:

Ufficio dello Stato Civile
Comune di Termini Imerese
Via Garibaldi
90018 Termini Imerese PA
Italy

should do it.

The comune has a website at http://www.comune.termini-imerese.pa.it/ however it appears to be down at the moment.
