Michael J. Duralek Sr., 68, of Waretown passed on Saturday, June 6.
Viewing is from 2 to 4 and 7 to 9 p.m. on Monday at Barnegat Funeral Home, 841 W. Bay Ave., Barnegat. Mass will be offered on Tuesday at 12:30 p.m. from St. Mary's Church, Barnegat. Private cremation to follow.
Born in Elizabeth, Mr. Duralek moved to Waretown from Berkeley Heights three years ago.
He worked as a chemical operator for the Monsanto Co., Kearny, for over 30 years.
Mr. Duralek is survived ... four brothers, Walter and Thomas Duralek, and Joseph and Edward Izworski, and two sisters, Mary Golen and Joan Betts.
In lieu of flowers, donations to the American Diabetes Association would be appreciated.

The SS-5, or Application for Social Security Number is a great resource for learning more about individuals who died after about 1960, and generally includes the following:
- Full name
- Full name at birth, including maiden name
- Present mailing address
- Age at last birthday
- Date of birth
- Place of birth
- Father's full name
- Mother's full name, including maiden name
- Sex
- Race as indicated by the applicant
- Whether the applicant ever applied for Social Security or Railroad Retirement before
- Current employer's name and address
- Date signed
- Applicant's signature
