Information on Sam Millonzi Faraci

Sam was born in Montemaggiore Belsito (Palermo Prov.), Sicily on 3 March 1890.
I wish to know when and with whom he left with to USA.
Where was his departure? What was the date? When he arrived in the USA and at what port of entry did he arrive? What was the name of the ship on which he crossed the Atlantic?

Possible match for a Santo Millonzi leaving Montemaggiore Italy on line 27. He was leaving with his brother a Salvator Millonzi line 28

The death record is correct. It matches to the Death Certificate i have.
However, the information on Santo & Salvadore are Sam's cousins.
Sam had to be in the USA prior to 1905, since he married Eva DeLorme and had a son, Francis J.Faraci in 1911.

Both Frank Faraci (B. 1847 & his wife Rose (B. 1867) are the parentsof Sam(uel) Faraci who lived with his son Rosso ( Russel) Faraci in Evans, NY. Both Franf & Rose had seven (7) chidren. Sam(uel) Faraci, whom I am interested in, lived in Buffalo and did some militray service in Connecticut. He lived in Chicago until his death and used his mother, Rose's maiden name - Millonzi - thus, Sam Millonzi (Faraci). The same person. That was his draft registration card WW I.
I believe that he and his entire family came over from Sicily at the same time and on the same ship and entered the USA. Please check his parents - Frank (Francseco) Faraci & Rosie (Rosalia Milllonzi) Faraci for the departure and with whom; as wlll as the port of entry in the USA - time and date and name of the ship.
