On Carminella death record, her mother surname is Rosa. The correct one is De Rosa.
When Carminella married she had to produce a birth certificate and supposedly her mother surname was correct! On the other hand, the death record is formed based on the knowledge of the individuals that declare the death to the town hall, and some times the knowledge is very poor...
