I don't know about Philidelphia, but the New York City consulate requires living ascendants to complete/sign this form http://www.consnewyork.esteri.it/NR/rdo ... /form3.pdf
For deceased ascendants, the applicant (you) submits this form on their behalf http://www.consnewyork.esteri.it/NR/rdo ... /form4.pdf

I think they will permit some sort of workaround. Basically, the form requires your grandmother to certify "THAT HE/SHE HAS NEVER RENOUNCED ITALIAN CITIZENSHIP BEFORE AN ITALIAN AUTHORITY".
If your grandmother had actually done so, there would be a record of it, so - at most - having the self-certification allows the consualte to not bother checking further.
