Translate so I can use it as a template for others

I have a birth record from Tuscany which is different from the ones I have used in Calabria. Please translate the document so that I can use it as a template as I move to other documents. I made two files... one where the top is better, and one where the bottom is better, but I was unable to get a good SINGLE one... Thanks for your help.

Thanks, Livio. This will help. It seems at first glance through the microfilm that many of the documents have a commentary at the end like this one has. I notice that there is no age for the mother. Appreciate your time.
