Name of the deceased, aged, profession, living in....., son of ..... and of....,
husband of ..... (or widow of , or single).

For example, Francesco's atti di nascita gives the cognome as "Di Iacovo".

The atti di nascita of his first-born son, Michele, is Di Iacovo.

Two years later, his second son is Filippo Iacovo, without the prefix.

Twenty years later, Filippo's first daughter is Carolina Iacovo.

After three more years, Filippo's second daughter is Rosina Di Iacovo.

All of these are directly from the Italian atti di nascita.
