The LDS microfilm begins in 1866 in my grandfather's comune in Sardinia. Does that mean there are no civil records before then? For any records before 1866 are parish records the only option?

You need to study the history of Sardinia. Who was ruling before 1866. Napoleon started the civil records in 1809 in the areas he conquered. I don't know if Sardinia was one of them. If it came under the French the records were probably sent to France.
