At 4 PM on 21 November 1849 before Stefano Simeone, Mayor of the Comune of Cervinara, District of Avellino, Province of Ultra, appeared Francesco Cillo, age 27, born in Cervinara, profession of laborer, living in Cervinara, son of Pellegrino Cillo, profession of laborer, living in Cervinara, and Leresa (?) Hignicolo (?), living in Cervinara AND Maria Angela Viola, age 25, born in Cervinara, daughter of Antonio Viola, profession of laborer, living in Cervinara, and Rosa Sancelia (?), living in Cervinara

Witnesses: Michilio (?) Cioffi, age 40, landowner, living in Cervinara
Francesco Racssci (?), age 50, landowner, living in Cervinara
Sirain (?) Monagucci (?), age 70, landowner, living in Cervinara
Raffaele Bruno, age 30, landowner, living in Cervinara

The right side of Image 1, the parish info is blank.. what does that mean?

what you have as Leresa is Teresa

the witness Cioffi's first name is Marcilio

Ragucci is Girolamo's surname (this is the witness you identified as Sirain (?)
Francesco's surname is the same.

I think Rosa's surname is Guerrieri and that what comes after that is not part of the surname.

I see there are people on the internet with the surname, and one or two people on Ellis Island who came from Naples with it, but I don't see the surname in the current Italian phonebook.

Are there indices for this town's records? Sometimes looking in an index at the various surnames also helps to know if the surname is correct, or even if the first letter of the surname is correct. I think it's the right surname, but if it's rare in this town's records, then maybe another record will indicate if this person was from somewhere else.
