Dated Feb 25, 1894
Informant, baby’s father, Francescantonio F., age 26, was a peasant/farmer in Frigento. He reported that, at 5:20 a.m. on the 23rd of the current month, a female infant named Filomena, was born in the house at 2 Contrada San Marco, to his wife, Mariantonia Di Leo, who was living with him. She too was a peasant/farmer.
Witnesses to the declaration of the birth, and presentation of the infant, were Francesco Caviglia, age 40, a telegrapher, and Giuseppe Mugnai, age 37, a property owner.
The witnesses and town official signed the document below. The informant, however, did not sign, as he was illiterate.

Also, where on the record, does the mother's name appear? That would help sort out relatives without bothering anyone on this forum. I did not know my surnames were so common in Italy.
