No, it doesn't mention parents' mothers.
Salvadore Pagano son of deceased Leonardo, and Giuseppe Di Maria son of deceased Giacomo are the witnesses who were present at declaration.
As to the other names you mention, I think you misread some Italian word: "dichiara" ("he declares") where it states "dichiara di dare il nome di Caterina"; "facchino" ("porter") is the occupation of Giuseppe Di Maria; "data lettura" ("after reading") in the fourth line before the end.
