How to address letter to Italy?

I'm sending a letter to all potential relatives (Ferraresi) in Italy but I'm confused on how to address the envelope. The phone book reads as follows:

Ferraresi Gianfranco
25035 Ospitaleto (BS) - Vis Dominico Ghidoni, 95

Then the phone number follows.

Does anyone know the format to send mail?

Sorry to ask such a ridiculous question, but I'm very new, but focused on this.

There are no stupid questions so don't be afraid to post them. Thats what this forum is all about.

The correct way to address this envelope would be:

Ferraresi Gianfranco
Via Dominico Ghidoni, 95
25035 Ospitaleto (BS)
Italia

If you forget to put Italy or Italia it will come back to you..I've heard it happen before.
