Who fills out the manifest?

I have been able to obtain a few manifest for my family so far. I am unsure of who actually fills them out and how they get the info.

Would appreciate finding out about the process if someone knows.

Someone has also made a statement about the markings on the manifest for one of my grandmothers, as having something to do with the her getting her naturalization papers.

Do they actually go back to the manifest and append it with things like that?
