I did google translate so I understand what it all says but what I don't understand are the forms, what are they for? (it seems as if you create your own) http://www.comunescalettazanclea.it/moduli.asp

Autocertificazione = Self-Certification

In Italy, in most cases, instead of presenting (when required) an official act you can fill in Self-Ceritification document.

As an example, when you ask for a passport it is sufficient that you declare, in writing, to the police that you are an Italian Citizen and that you were born in xxxxxx on mm/dd/yyyy. You do not need to show an official document.

The site that you mention allows you to create your own forms.
