PHILADELPHIA- For my upcoming J.S. appointment, will I need to fill out the "Modulo Multifunzionale" form?

I'm also curious about the "Cittadinanza Jure Sanguinis" form. Which seems redundant given all of the other forms I need to fill out that contain similar information.

My situation GM ----> F ------> ME (Please note, my father has died, while my grandmother is still living)

I have acquired all of the necessary documentation, translations and apostilles, but these other random forms are stressing me out.

Actually.....can someone PLEASE give me a list of the documents that I will need to fill out before my appointment at the Philadelphia Consulate?
