G-639 Question

G-639 Question

I'm filling out a G-639 to request naturalization records. I'm in pretty good shape in that I have a copy of the Certificate of Citizenship with enough information on it to request a certified copy of the naturalization records. But I'm not sure how to fill this form out.

Specifically, I've checked the box stating that the individual is deceased. I've included a photocopy of the death certificate (or do I need to include the original certified copy?). I assume that section 3 remains empty since there is nobody alive to sign it. I don't know what to do with sections 6 and 7 though. Since I am not requesting my own information, do I need to fill out section 6? What about section 7 and the notary? The instructions on this thing are not clear at all.

Can anyone offer me some help?
