The anagrafe office knows everyone in town so even if they know that a child is being raised by its natural parents, they will still abide by the law and fill out forms with the 'official' information. If that child ultimately leaves the town where he was born/raised you have a small chance that the official death record for example will reflect the true parents, if they were known to the person declaring the death. If they die where they were born only the official information will be recorded. You have to be incredibily lucky to trace birth parents and someone has to have known who they really were.
