Record transcription help needed

Is the fathers name Nicoletta Sica? Was that first name ever given to males? Also, I know the mothers name is Maria Teresa DiGuglielmo and she married a D'Ambrosio. Did this child, Vitantonio D'Ambrosio take the name of the husband?

If someone is better at reading this document it may help to clear things up. It's #46, top left. Thanks!

Nicoletta Sica, a midwife, was the declarant. Angelo d'Ambrosio, Vitantonio's father, could not report the birth, because he was in America
