Family name confusion

Hello, I am looking for a starting point. My grandfather was born in NY. His parents were from Padula. There is a story that when my great, great, grandmother was giving birth that the midwife filled out the papers wrong so now half of my family had the last name Bianco and the other half had Pandolfi.

There are a lot of documents available from Padula.
Can you give us some names and dates?

The midwife doesn't fill out the papers. She might declare the birth at City Hall in which case the child might be given the surname of the mother if the parents weren't married 'civilly'. If the father declared the birth they would be given his surname.
