1866 Death Record-Antonio Girardi

Would like to verify the following information:
Record #14: Reported to Giovanni Gallo, Mayor on 19 Jan 1866, death of Antonio Girardi, age 25, living in Cervinara. Father: Nicola Girardi Mother: Fortunata Cioffi Spouse: Grazia Casale
Witnesses: Francesco Esposito, son of the deceased Pellegrino, age 39, profession of laborer, AND Domenico d ?, son of the deceased ?, age 35, profession of laborer.

Also, what is the actual date of death?

looks to me like it was the 18th of the same month (two lines above his name).

Nicola is deceased at the time of his death.

Domenico maybe De or Del Gregorio. Father's name looks like Vincenzo

the image is rather blurry to me, so I cannot be accurate with spelling of the names. sorry i could not be of more help.

Yes, I agree the image is a little blurry. That is the best I could get this film focused.
