Looking to verify the time and the day of birth: 6 this morning- 16 October

i think the month is december. at the end of the first line it looks like "Di" with a marking under the 'i' to indicate the word is continued on next line. and the next line begins, i think, 'cembre'. i also think that the hour of birth was 6am.
