Translation, please, for death act notation

I'm hoping to clarify some of the writings on various death acts. So in the linked image, there is the paragraph on the printed form, which I would like to understand better, and also the handwritten notation of the scribe, which seems to indicate some accommodation to not having a written record?

The civilian officer, along with the witnesses, personally verified the death. He then compiled two copies of the act of death and read it to witnesses. Then only the Officer signed the act, because witnesses claimed to be illiterate ("avendo i testimoni asserito di non sapere scrivere")
