Montefalcone Italy / Formato Family

Hope someone can help me, I have been searching for over 2 years and now have reached a dead end because of conflicting records. I am researching Joseph Formato appr. birth around 1878 in Naples or Benevento Montefalcone area.
He was a tailor and had a brother named Louis. He married a D'Alessio and I was told they were neighbors. I have written to Comune Di Montefalcone Di Val Fortore and they did reply in Italian to say they regret to find nothing in their files. Ive been on Ancestry and Ellis Island sites and neither were helpful. He lived in Manhattan, NY but later moved to Chicago according to various census records. I am hoping to solve the puzzle and find the missing pieces. There are no living relatives or any records kept by them to help me. His daughter, Mary, age 92, passed away last year and no notes or records were found.

Is this the correct record?
To confirm his place of birth, have you found his ship manifest or naturalization paperwork?
(1920 census indicates this Joseph filed his 1st papers)

Since Joseph probably gave the information, you would think that Montefalcone Di Val Fortore would have his birth record. Unless, as he stated on his WWII Card, he really didn't know when he was born?

I found the 1930 Census for Consolata/Concetta and kids in Chicago. No Joseph.
She's listed as "widow"... take a look. Maybe it's a different Formato family?

Or maybe the WWII card is the wrong Joseph?

You're right :D it's the same family in 1920 and 1930 - good catch
I'm guessing the 1937 death record and 1942 WWII Draft Registration Card are for someone else?

Found a manifest for Agostino D'Alessio, Giuseppe's brother-in-law, with whom he and Consolata were living on the 1900 Census above.

Going to wife and kids in NYC
Last place of residence: Montefalcone Val di Fotore, Italy

That means that Consolata D'Alessio (Formato) was probably also from there. Maybe a new letter to the comune can produce a marriage record (according to the census was in 1899) which will indicate Giuseppe's place of birth... UNLESS they were married in US :-(

Thanks to all who are trying to help. Joseph (my grandfather) married Consolata D'Alessio also and they had 5 children Carlo Michele (Charles) born Nov. 1900 in NY, Ann(Anne)
Leonard, William (My Father) and Mary (92 yrs old and last living sibling.
I saw the same records that have found but no manifest either.
In later census reports, it shows Consolata as head of household.
On the death certificate for Joseph it states his mothers name as Mary., his mother couldn't be his wife. Maybe he didn't understand the question.
Consolata relates to the virgin Mary, maybe they called her Mary for short.
his death certificate is very confusing. I do appreciate all your help.
